Application Components

Your application should include the following:

Executive Summary (500 words)

Please provide a short overview of the proposal.

Statement of Need (600-700 words)

Please describe the need your project will address. Include relevant data that demonstrates this need in your community

Project Overview (250 words)

Provide a short overview of the program, services, or activities you seek to fund.

Goals and Objectives (750 words)

Please provide a description of the goals that your project will achieve, along with the specific results or objectives that will help you meet that goal. Your goals and objectives should be SMART (Specific, Measurable, Achievable, Relevant, and Time-bound)

Project Description and Method (1250 words)

Describe the project in detail, including the methods you will use to meet your goals. Please include a project timeline.

Evaluation Plan (750 words)

Please tell us how you will know if your project was successful. Explain how you will evaluate the impact of your work.

Budget and Budget Justification (500 words)

Please provide a detailed program budget, including project expenses and any projected revenues. Please include a narrative explaining each line item in the budget.

Organizational Information (500 words)

Provide an overview of your organization’s experience and history. Please explain why you are qualified to carry out this project, and provide details about your experience working in the community you seek to serve.
